Hospice Chaplain/Bereavement Coordinator
The Hospice Chaplain/Bereavement Coordinator is responsible for the provision of spiritual care services to patients and families/caregivers either directly or through coordination of care with other spiritual counselors. To provide supportive services to help meet the needs of the terminally ill hospice patient and family as needed. To provide assistance and understanding to the family in time of bereavement. Responsible for developing, implementing and supervising the bereavement program and for the delivery of related services.
Duties and Responsibilities
· Assures spiritual assessment of patients and families/caregivers in the hospice program and appropriate services are rendered in a timely manner.
· Manage the bereavement services program utilizing professional staff and volunteers.
· Facilitates the development of the individualized plan of care by participating in the comprehensive assessment to meet identified spiritual needs.
· Provides direct spiritual care to patients and families/caregivers.
· Provides consultation, education, and support to the interdisciplinary group on spiritual care.
· Plans periodic memorial services to meet the needs of personnel, volunteers, and community clergy/spiritual counselors working with the hospice team.
Qualifications:
· Graduate of accredited seminary or school of theology preferred (BD, MDiv or equivalent theological degree).
· Masters degree required.
· Experience working with Hospice / Medical experience; individuals, family, or caregivers preferred.
· Must have experience or education in grief or loss counseling. Active patient contact within past three years, preferred.
· Ability to respond appropriately to crisis outside of a hospital setting.
· Acceptance of and adaptability to different social, racial, cultural and religious modes.
· Must have a current valid driver’s license, auto liability insurance and reliable transportation.
