Job Description

Job Duties:

Core Responsibilities

· Update trackers on the following medical coordinator tasks:

· Cart audit

· Hospitalization/discharge documents

· Appt records

· LOA form

· Weights

· Use live spreadsheets to document findings from audits and share with medical management team.

· Report any emergent/patterns of inaccuracies found in audits

· Audit Med team calendars and check FlatRock Information Network that appointment records are uploaded

· Audit appointment records

· Monitor residents’ weights and report fluctuations to medical management team

· Responsible for Flatrock Inc. residents arrive to their medical appointments on time

· Update morning team post in medical thread and update planner

· Update Direct Care Worker/Lead tracker after Primary Care Physician training, home orientation #4 training.

· Build Psych & Injections schedule

· Coordinate with Pharmacy regarding prescription refills, insurance issues, etc. and audit all chart reviews

· Monitor and respond to med team email

· Build, maintain, and update psych schedule (both internal and eternal), injections schedule (both internal and external), monthly PCP schedule, and Health Care Appraisal

· Send updated HCA’s/Injection appointment records to coordinator

· Send HCAs to operations when signed off on by PCP biweekly

· Send HCA’s/Injection appointment records to coordinator

· Send injection list to support team

· Send support team medical reviews

· Enter injections into Medication Administration Record

· Send non billable report weekly on Thursday’s

· Verify Coordinators has injection for outside provider before appointment

· Monthly send template of PRN /Control logs to medical coordinators

· Monthly review completed PRN/Control logs, upload to sharepoint, and share data with medical management team

· Maintain to do list

· Send discharge paperwork to PCP to ensure follow up visits are being completed.

· Referral follow-up: ensure they are in FIN and appt has been scheduled and added to calendar

· Follow up on discharge paperwork to ensure medical coordinators are completing all discharge instructions in a timey manner

· Follow up on all lab orders to ensure medical coordinators are completing all orders in a timely manner

Be flexible and commited to work the hours and times required to perform the responsibilities of the job, including evenings and weekends as may be necessary

Perform administrative tasks, and act as a liaison between medical staff, residents, and administrators.

Effective Teamwork & Communication

· Maintain a positive, open relationship with all management and staff 

· Utilize all communication tools including FIN

· Maintain and advocate for all Flatrock Inc. residents’ rights  

· Complete any additional tasks assigned from Healthcare Manager
