Physical Therapist (PT) – Assisted Living & Home Health | Des Moines, IA

We are seeking a Physical Therapist (PT) for a full-time (30 hours per week) opportunity covering Assisted Living and Home Health settings in Des Moines and Central Iowa . This role offers a flexible work environment , a strong support system, and opportunities for mentorship and career growth .

• Patient Care: Evaluate, develop, and implement personalized treatment plans to enhance mobility, function, and overall well-being.
• Therapy Services: Provide hands-on therapy in Assisted Living and Home Health settings, ensuring optimal patient outcomes.
• Documentation: Maintain accurate patient records, progress notes, and treatment plans in compliance with state and professional standards.
• Collaboration: Work closely with patients, families, physicians, and care teams to ensure a comprehensive and coordinated approach to rehabilitation.
• Program Development: Help plan, market, and implement new therapy programs to better serve patients and the local community.
• Education & Training: Conduct in-service training sessions for fellow staff therapists to promote ongoing professional development.
• Quality & Compliance: Ensure all therapy services adhere to facility policies, procedures, and regulatory requirements.

• Education: Bachelor’s, Master’s, or Doctorate (DPT) in Physical Therapy from an accredited program.
• Licensure: Must possess or be eligible for state licensure as a Physical Therapist (PT) in Iowa .
• Experience: While prior experience is preferred, new graduates with a strong commitment to professional growth and patient care are encouraged to apply.
