Job Description
We are seeking a dedicated and skilled Registered Nurse (RN) to join our client’s Outpatient Clinic team. In this role, you will provide direct nursing care to walk-in patients, coordinate treatment plans, and ensure a seamless connection with our affiliated inpatient hospital. This position requires clinical expertise, excellent communication skills, and a commitment to delivering high-quality patient care in a dynamic and supportive environment.
*Key Responsibilities:*
* Initial Assessment: Conduct triage and comprehensive assessments of walk-in patients, gathering medical histories and identifying immediate care needs.
* Direct Care: Provide nursing care, including wound care, medication administration, vaccinations, and other treatments as prescribed.
* Care Coordination: Develop and implement care plans in collaboration with patients, physicians, and other healthcare professionals, focusing on holistic patient care.
* Patient Education: Educate patients and their families on health conditions, treatment plans, medication management, and preventive care strategies.
* Crisis Management: Respond to medical emergencies, initiate appropriate interventions, and facilitate emergency care as needed.
* Interdisciplinary Team: Collaborate with physicians, nurse practitioners, social workers, and other clinic staff to deliver integrated care and optimize patient outcomes.
* Inpatient Coordination: Work with the affiliated inpatient hospital to ensure smooth transitions of care for patients moving between inpatient and outpatient services.
* Referral Management: Facilitate referrals to specialists, diagnostic services, and community resources as necessary.
* Accurate Records: Maintain accurate and timely documentation of patient assessments, care plans, interventions, and outcomes in the electronic health record (EHR) system.
* Compliance: Ensure documentation complies with organizational policies, regulatory requirements, and best practices in patient care.
* Best Practices: Participate in quality improvement initiatives to enhance patient care, clinic operations, and adherence to clinical guidelines.
* Patient Satisfaction: Monitor and address patient satisfaction, striving to improve the patient experience and meet clinic goals.
* Patient Outreach: Participate in patient outreach activities to promote clinic services and health education in the community.
* Professional Development: Engage in continuous learning and professional development to stay current with nursing practices, clinical guidelines, and healthcare innovations.
*Requirements:*
* Bachelor’s degree in Nursing (BSN) from an accredited nursing program.
* Current, unencumbered Registered Nurse (RN) license in NY.
* Previous experience in an outpatient, urgent care, or emergency department setting preferred.
* Proficient in clinical assessment, critical thinking, and patient care. Strong interpersonal and communication skills.
* Knowledge of outpatient care protocols, infection control, and emergency response procedures.
* Compassionate, detail-oriented, and capable of working effectively in a fast-paced environment.

Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer._
